Turns out it is likely a Telco problem. A few days after I switched 
to 64k, the 64k calls started failing in the same way - dial, 
connect, no authentication. Trouble calls to the ISP narrowed it down 
to people calling a particular exchange from outside that exchange. 
GTE got on it and now the 64k is back and the voice connects work 
about 50% of the time - still not enough for me to switch back to 
unmetered DOV, unfortunately.
