You are omitting the following *REQUIRED* attribute:

Without it, the MAX will never attempt an outgoing call.
FWIW, you can also use 'Dialout-Framed-User'. This also works with callback,
but I presume you can also do dialout (never checked, though).
The rest of your profile looks all right, but...
I have a related problem with callback using CLID (no, not to a cisco). It
works like a charm with Internal profiles, but not with radius (on a Max, with
5.0Ap7)
When I use a P400/E1 (4.6Bp3) the Radius CLID-callback does work (same
radius-server/users-file).
Any suggestions ?
