Disconnect cause code 11 is not necessarily abnormal. It just means that
carrier dropped before the MAX was told the user was disconnecting.

Check the syslog or RADIUS accouting files.
Is there a correlation of the Disconnect Cause and particular users,
modems or lines on the MAX?
Are the sessions that end with carrier dropping typically shorter sessions
or do they seem to be similar in length to other sessions?
