I've seen this only on oue of our boxes that has
mainly perm/switched outbound dialed links,
two PRI going to a NI-2 Siemens switch.

35 channels are expected to ALWAYS be connected,
and sometimes one particular customer with a 3COM router
(it always starts with them, then gets worse)
will have one channel disconnect, and the link drops on the 
customer end, but never clears on our side, the max dials out 
again on an available channel. eventually all channels are full 
from our point of view, even though there should be ~10 free.

It happens less than once a month, and is usually
solved my disabling and reenabling the trunks.
