I belive CONFIG_X86_LOCAL_APIC is actually defined in the kernel
configuration files, but I'm not positive. What are the results if you
drop the #ifdef/#endif sets there? So something like this:

#include <linux/config.h>
#include <asm/fixmap.h>
#include <asm/i82489.h>
#include <asm/bitops.h>
