Ugh! forgot, also will need domain name or static IP(s) for the firewall. 

For the static IP(s): N/A is an acceptable answer. For instance if you are on
a dialup or Cable modem.

For domain name, it must be the domain name that your IP address reverse-name
lookups too. If you have a virtual domain name, that won't work, it must be
the IP address from your ISP.
