That does sound like a problem.  Are you using NFAS on your 4004?
With NFAS, since two or more PRI lines use the same D channel, an error
on the D channel can cause a failure of a large number of B channels.

45    PPP - Receive Terminate Req
185   Remote End Hung Up

Cause No. 16 - Normal Call Clearing
This cause indicates that the call is being cleared because one of the users
involved in the call has requested that the call be cleared.

Cause No. 102 - Recovery On Timer Expiry
This cause indicates that a procedure has been initiated by the expiry of
a timer in association with Q.931 error handling procedures.

The only one of the codes that look unusual is the ISDN cause code of 102.
