More info:

It appears it happens only when a call comes in several seconds after a
call has just cleared on the same line/channel:

May  8 20:27:08 m18-2 ASCEND: slot 2 port 1, line 5, channel 1, Call
Disconnected [MBID 209; ->6039240] 
May  8 20:27:08 m18-2 ASCEND: slot 2 port 1, Call Terminated [MBID 209;
->6039240] 
May  8 20:27:12 m18-2 ASCEND: slot 0 port 0, line 5, channel 1, Incoming
Call, MBID 222 [MBID 222; ->6039240] 
May  8 20:27:12 m18-2 ASCEND: slot 0 port 0, Call Refused [MBID 222;
->6039240] 
May  8 20:27:13 m18-2 ASCEND: call 91 CL 0K   c=9 p=2
May  8 20:27:13 m18-2 ASCEND: slot 0 port 0, line 5, channel 1, Far End
Hung Up


Notice 4 seconds before the call Refused, line 5 channel 1 disconnected.
