Packet loss above 3% somewhere along the line is almost always the cause
of this (which can be caused by the above list); I find a good test is to
do a traceroute to your mail server and do ping -q -c 100 aaa.bbb.ccc.ddd
on each hop starting with the first to see where it is (since it may be
further than just the next hop).

ie: (This isn't from a MAX, but the commands work the same)
