That "far end hung up" msg make me think in our problem with MAXes.
Here is the line:

Feb  9 10:52:30 max-xxx ASCEND: slot 0 port 0, line 1, channel 29, 
 Far End Hung Up, Cause code 047

Looking at the modem status, sometimes, a bunch of them are in 'ck'
status, which appear to be in 'Checksum' status (cannot be reset
between calls).. We highly suspect of the PRIs, and reported this
to the telco.

The only temp solution is resetting the MAX (sigh).
