This is a very non-technical answer. So someone who knows the specs should
jump in if they want.

As far as 56K modems go, the ISP's all had to set up shop with specific
hardware that allowed them to "upload" at the faster speed. IOW, they
didn't have the same 56K modem that you or I would've bought at CompUSA or
wherever. Plus they had some kinda digital connection to the telco on
their end. On the other hand, if you tried to communicate with me and we
both had, say, USR 56K modems, we'd be limited to the slower speed.
