What version of software are you using on your MAX 4000?

Your RADIUS user entry seems to closely match the documentation examples
and you are seeing the User-Name in the Access-Accept, so it would appear
you are doing things properly.

You should probably start the bug report process with Ascend, assuming
you are using a supported version of software.

As a work around, you might try adding the Ascend-Authen-Alias attribute
with the name you wish the MAX to use.  Ascend-Authen-Alias is typically
only used for incoming calls to tell the MAX to use a different name.
