Actually, I've seen similar behaviour in a MAX4004 with 4 T1's, 3 being
PRI from one telco, and 1 was AMI/CT1 from another - turns out there were
clocking issues that was causing all sorts of bizarre behaviour (not to
mention that the box was overloaded, which probably excaberated the
problem). Calls would make it to the MAX according to the telco, but the
MAX would refuse them without any indication in the status windows that a
call was coming in; the user would hear a slight pause when dialling in,
followed by a fast busy.  Solution? We moved the CT1 onto a different box.
Next time it happens, perhaps you could try unplugging the T1's one at a
time to move the clock source around (use the diagnostics clocks command
to see what source is being used) - when you start getting refused calls,
you'll know you've got the right one.

This is an extremely painful process, especially when the box in question
is a dedicated ISDN box, not to mention time-consuming.  I'd use it as a
last resort (especially since the problem may lie somewhere else).
